Home Health Care

Skilled care helps patients be independent and safe in managing their care at home. Nurses provide medical care and coordinate the services of rehabilitation professionals, social workers, and health aides. Care plans and education are tailored to the specific needs of each patient and family.

Hospice Care

Compassionate care for individuals and families designed to care for those at the end of life who no longer wish to seek treatment and cures for their terminal illness. We tailor our team approach to the needs and wishes of each patient and family, so they live life to its fullest. We address physical, emotional, social and spiritual challenges, to help you find peace and comfort.

Bereavement Support Care

The groups will help explore and understand better the grief process and how to celebrate and honor the loved ones we have lost. The groups meet weekly for 8 weeks and are held several times throughout the year.

Palliative Care

A program of healthcare services designed to care for those with life limiting conditions that wish to continue to seek treatment and/or cure, but who also want to maintain satisfying quality of life and manage their symptoms. The VNA of Manchester and Southern NH work with Elliot Palliative Care Service in proving for Palliative Care Medical consultations.

We are here to help, seven days a week. We provide a wide range of skilled and loving care to ensure that you and your child receive the care and support you need

Referral Capacity

Our services may be limited by:


Staffing and Service Intensity

We continuously monitor our census, staffing and clinician caseloads and complexity to determine referrals which we have capacity to accept. This can change day to day. There may be times where there are longer waits for a particular service (nursing, physical therapy, occupational therapy, speech therapy, social work, licensed nursing assistant).


Based on our current staffing levels as of December 2024:

  • We are unable to support daily or more frequent services without a predictable end date and/or patient/caregiver willingness to learn.
  • We are unable to support twice daily Licensed Nursing Assistant services



Service Complexity

We are unable to provide the following services:


Nursing

  • Blood/blood bi-products (IV Immunoglobulin)
  • Nursing to sit through lengthy (1 hour or more) infusions (Patient or caregiver must be able to be taught).
  • Disposable Negative Pressure Wound Therapy (dNPWT)
  • Acute psychiatric or behavioral health nursing


Rehab

  • Lymphedema Therapy
  • Aquatic Therapy
  • Pelvic floor Therapy
  • Certified clinicians in Lee Silverman Voice Therapy (LSVT)
